This PhD co-designed, implemented, and evaluated a Family Systems Care (FSC) programme across the perinatal continuum in Ghana to strengthen family involvement and improve care experiences.

Using a participatory action research framework, the project progressed through four phases: a scoping review and stakeholder engagement; baseline data collection with families and healthcare professionals (HCPs); development and delivery of an FSC training programme; and implementation through systemic family–HCP conversations.

The review revealed limited evidence and the need for culturally adapted FSC frameworks. Baseline findings showed inadequate family support and substantial training gaps among HCPs, despite strong interest in FSC. Thirty HCPs completed an 8‑day training and co-developed a context-adapted pocket guide. Implemented conversations enhanced shared decision-making, conflict resolution, and cognitive support, though emotional needs remained insufficiently addressed.

Overall, FSC proved feasible, acceptable, and empowering, offering the first evidence from Ghana and supporting its scalability within the health system.
