Abstract

Phakic intraocular lenses stand as a safe and effective modality for patients with refractive errors beyond the range of corneal surgery or those seeking a reversible solution. The current state of evidence indicates high satisfaction and visual performance, with cataract formation (in ~10–20% of eyes over 10 years) and chronic endothelial cell loss (on the order of 1–2% per year for iris-fixated lenses) being the primary long-term concerns. With meticulous patient selection, proper sizing, and regular follow-up, many of these risks can be mitigated. Ongoing innovations in PIOL design and measurement methods are expected to further improve safety – potentially making phakic IOL implantation an even more attractive option for vision correction. For the clinician, staying informed of these developments and understanding the nuances of studies like those by our team is crucial for counseling patients and making evidence-based decisions. Ultimately, when used in the right setting, phakic IOLs offer a compelling balance of efficacy (crisp vision), safety, and reversibility, contributing significantly to the refractive surgical armamentarium in 2025 and beyond.
