Abstract

Pediatric patients present a unique challenge in the management of bile duct disorders, in which small-sized structures and their developing physiology highlight their increased susceptibility to complications. The surgical management of pediatric hepatobiliary disorders not only requires thorough preoperative evaluation and meticulous surgical intervention but also carries a significant risk of postoperative complications.

This thesis encloses a compilation of relevant scientific studies designed to explore the complexities and challenges in diagnosing, managing, and treating pediatric patients with bile duct disorders. Key points from this research highlight the intricate nature of pediatric bile ducts making them particularly susceptible to complications during and after surgical interventions.

Thus, several studies are reviewed including case series, consensus-built Delphi method studies and uni/multicentric retrospective cohort studies, which contribute to a better understanding of the complications that can arise from pediatric bile duct disorders. The need for precise diagnostic criteria for improving surgical outcomes is emphasized in managing post-Kasai cholangitis for biliary atresia patients. Furthermore, the need for effective treatment strategies to deal with frequent postoperative biliary complications in the setting of pediatric liver transplantation is underlined.

The implications of the findings for clinical practice are discussed, underscoring the need for enhanced diagnostic bundles and management approaches that accommodate the unique challenges of the pediatric bile ducts.

In conclusion, this body of work contributes to the field of pediatric bile duct disorders by proposing solutions that can lead to better patient outcomes. The findings encourage ongoing research to refine management strategies to improve the quality of care and the quality of life for pediatric patients facing hepatobiliary surgery.
