Challenges and perspectives in the diagnosis of pulmonary embolism in specific populations of patients

Abstract

Current diagnostic management of patients with suspected pulmonary embolism (PE) relies on diagnostic strategies, rather than an isolated diagnostic test, with sequential assessment of pre-test clinical probability, D-dimer measurement, and – only when needed - thoracic imaging. This work first presents the evolution of PE diagnostic algorithms and challenging issues in specific subgroups of patients. Then, some of our studies contributing to fill knowledge gaps in some subgroups are presented: analysis of the usefulness of D-dimer in patients with renal impairment, prospective validation of diagnostic strategies and assessment of their safety in pregnant patients, assessment of the safety of multidetector CTPA to exclude PE in patients with likely clinical probability. Finally, challenges and future perspectives to overcome concerns about the overuse of CTPA are presented, such as the potential development of PE clinical suspicion rules applicable to all patients with chest symptoms, and the development of alternative less radiating imaging tests.
