Bioméd. - Méd. légale 8 Cette thèse, intitulée « CT-angiographie postmortem chez le fœtus et l’enfant : développement, revue systématique et apports médico-légaux à partir de cas pédiatriques », explore l’application des techniques d’imagerie postmortem, en particulier l’Angio-CT postmortem (PMCTA), dans l’investigation des décès fœtaux et pédiatriques. Alors que l’imagerie en coupe s’est progressivement imposée comme complément, voire, dans certains contextes, comme alternative, à l’autopsie conventionnelle en pathologie médico-légale, son utilisation chez le fœtus et le jeune enfant reste un domaine émergent, marqué par des défis à la fois techniques, anatomiques et interprétatifs. L’objectif de ce travail était de développer et d’optimiser des protocoles de PMCTA adaptés aux spécificités de ces populations vulnérables, tout en tenant compte des considérations éthiques propres à l’imagerie postmortem des mineurs. La thèse repose sur trois articles originaux qui développent, synthétisent et évaluent des méthodes dédiées. Une assistance à la relecture linguistique a été réalisée à l’aide de ChatGPT (OpenAI, 2024–2025) ; cependant, l’auteure demeure seule responsable du contenu scientifique et des conclusions. Le premier article, consacré au « Développement d’une technique de CT-angiographie postmortem du fœtus avec un agent de contraste lipophile », décrit la mise au point et la validation d’un protocole innovant permettant, grâce à un agent lipophile mieux adapté aux modifications vasculaires postmortem, d’obtenir une opacification vasculaire supérieure à celle des produits de contraste solubles dans l’eau utilisés chez les patients vivants (produits de contraste iodés hydrophiles). Ce protocole s’est révélé reproductible et fiable pour la visualisation des principaux vaisseaux fœtaux, offrant ainsi une base solide pour l’intégration de l’angiographie dans l’investigation des décès fœtaux. Le deuxième article, intitulé « CT-angiographie postmortem des fœtus, nourrissons et enfants jusqu’à 12 ans : revue systématique et arbre décisionnel conceptuel », propose une synthèse structurée de la littérature existante sur la PMCTA pédiatrique. Au-delà du constat de la faible standardisation des pratiques, cette revue a abouti à la proposition d’un arbre décisionnel guidant le choix des protocoles selon l’âge, le poids, l’accessibilité vasculaire ou encore les objectifs diagnostiques. Ce cadre pratique constitue un outil de référence pour les praticiens. Le troisième article, « CT-angiographie postmortem chez le nourrisson et le jeune enfant (PedPMCTA) : faisabilité technique et apports médico-légaux à partir d’une série de 30 cas », a évalué empiriquement un protocole lipophile adapté au poids chez des sujets âgés de 1 jour à 3,6 ans. La faisabilité a été confirmée sur l’ensemble de la fourchette d’âge pédiatrique étudiée, avec une opacification artérielle et veineuse constante et des voies d’accès vasculaires fiables, en particulier la voie fémorale. Des shunts artérioveineux ont été observés dans certains cas, tandis que des structures cruciales en médecine légale, telles que le sinus sagittal supérieur, les veines ponts ou les vaisseaux oculaires, ont pu être visualisées dans une proportion substantielle des examens. This thesis, entitled “Postmortem CT Angiography in Fetuses and Children: Development, Systematic Review, and Forensic Insights from Pediatric Cases,” explores the application of postmortem imaging techniques, with a particular focus on Postmortem CT Angiography (PMCTA), in the investigation of fetal and pediatric deaths. While cross-sectional imaging has progressively emerged as a complement, and in some contexts an alternative, to conventional autopsy in forensic pathology, its use in fetuses and young children remains an evolving field, facing both technical and interpretative challenges. The objective of this work was to develop and optimize PMCTA protocols tailored to the anatomical and physiological specificities of these vulnerable populations, while taking into account the ethical considerations inherent to postmortem imaging in minors. The thesis is structured around three original articles that respectively introduce, synthesize and evaluate dedicated PMCTA methods. Language editing assistance was provided using ChatGPT (OpenAI, 2024–2025); however, the author remains solely responsible for the scientific content and conclusions. The first article, “Development of a technique for postmortem CT angiography of fetuses with a lipophilic contrast agent” describes the design and validation of an innovative fetal PMCTA (FPMCTA) technique. Thanks to a lipophilic contrast agent, better suited to postmortem vascular changes, this protocol achieved superior vascular opacification compared to previous water-soluble approaches (hydrophilic contrast medium used in the living). It provided reliable visualization of major fetal vessels and offered a reproducible method for integrating angiography into the postmortem investigation of fetal deaths. The second article, Postmortem CT angiography of fetuses, infants and children up to 12 years old: a systematic review and conceptual decision tree” presents a comprehensive synthesis of the existing literature on fetal and pediatric PMCTA. Beyond highlighting the current lack of standardization in clinical practice, the review led to the proposal of a decision tree designed to guide protocol selection based on age, weight, vascular accessibility and diagnostic objectives. This practical framework serves as a reference for clinicians and forensic practitioners. The third article, “Postmortem CT angiography in infants and young children (PedPMCTA): Technical feasibility and emerging forensic insights from a 30-case study” empirically evaluates a weight-adapted lipophilic PMCTA protocol in 30 individuals aged 1 day to 3.6 years. Feasibility was confirmed across the entire pediatric age range studied, with consistently high arterial and venous opacification and reliable vascular access routes, particularly via the femoral approach. Arteriovenous shunts were observed in several cases and key forensic structures, including the superior sagittal sinus, bridging veins and ocular vessels, were successfully visualized in a substantial proportion of examinations. Although artifacts such as contrast extravasation and air bubbles were encountered, they did not hinder diagnostic interpretation.
