A two-dimensional panoramic radiograph (PAN) showed a subtle radiopaque image apparently superimposed on the roots of the third molar, projected over the course of the inferior alveolar canal, suggesting of a close relationship between the inferior alveolar nerve (IAN) and the apices of tooth 38 evoking a nerve conflict. To further assess this apparent anatomical relationship, a three-dimensional cone-beam computed tomography (CBCT) scan was performed. The CBCT demonstrated no direct contact between the mandibular third molar and the inferior alveolar nerve canal. Instead, it revealed a well-defined radiopaque lesion adjacent to the nerve canal, separate from the tooth roots, consistent with idiopathic osteosclerosis. This case highlights the limitations of two-dimensional imaging in evaluating complex anatomical relationships in the posterior mandible. When panoramic radiography shows equivocal findings due to superimposition and suggests a potential nerve–tooth conflict, three-dimensional imaging is essential for accurate diagnosis and treatment planning. Introduction : La radiographie panoramique constitue l’examen de première intention pour évaluer les rapports anatomiques entre les troisièmes molaires mandibulaires et le canal alvéolaire inférieur avant leur extraction. Toutefois, les superpositions inhérentes à l’imagerie bidimensionnelle peuvent conduire à des erreurs d’interprétation. Ce rapport de cas illustre l’intérêt du CBCT dans le diagnostic différentiel d’une opacité radio-opaque simulant un conflit dento-nerveux. Matériels et méthodes : Un patient de 28 ans a été adressé pour l’avulsion des troisièmes molaires mandibulaires après plusieurs épisodes de péricoronarite. La radiographie panoramique montrait des signes radiologiques suggérant une relation étroite entre la dent 38 et le canal alvéolaire inférieur, associés à une discrète opacité radio-opaque apicale. Un examen CBCT à champ limité a été réalisé afin de préciser les rapports anatomiques. Résultats : L’examen CBCT a exclu tout contact entre les racines de la dent 38 et le canal alvéolaire inférieur. Il a mis en évidence une lésion radio-opaque homogène, bien circonscrite, d’environ 7,5 mm, située à proximité du canal mais indépendante des racines, compatible avec une ostéosclérose idiopathique. Les troisièmes molaires ont été extraites sans complication. En l’absence de symptômes, d’atteinte neurologique ou de signes d’agressivité, aucune biopsie n’a été indiquée et une surveillance radiologique a été retenue. Discussion : Ce cas met en évidence les limites de la radiographie panoramique pour l’évaluation des rapports entre les troisièmes molaires mandibulaires et le canal alvéolaire inférieur. Le CBCT a permis de lever une ambiguïté diagnostique liée à la superposition des structures anatomiques et d’orienter le diagnostic vers une ostéosclérose idiopathique. Une interprétation rigoureuse des examens d’imagerie et une démarche diagnostique différentielle sont essentielles afin d’optimiser la planification chirurgicale et de limiter le risque de complications neurologiques. Einleitung : Die Panoramaschichtaufnahme ist die Standarduntersuchung zur präoperativen Beurteilung der Lagebeziehung zwischen retinierten Unterkieferweisheitszähnen und dem Canalis mandibulae. Aufgrund der zweidimensionalen Bildgebung können jedoch anatomische Überlagerungen zu Fehldeutungen führen. Dieser Fallbericht zeigt den diagnostischen Stellenwert der digitalen Volumentomographie (CBCT) bei der Abklärung einer radiopaken Läsion, die einen Zahn-Nerv-Konflikt vortäuschte. Material und Methoden : Ein 28-jähriger Patient wurde nach rezidivierenden Episoden einer Perikoronitis zur Entfernung der Unterkieferweisheitszähne überwiesen. In der Panoramaschichtaufnahme fanden sich radiologische Hinweise auf eine enge Beziehung zwischen Zahn 38 und dem Canalis mandibulae sowie eine diskrete radiopake Struktur im apikalen Bereich. Zur dreidimensionalen Beurteilung wurde eine CBCT-Untersuchung mit kleinem Aufnahmevolumen durchgeführt. Resultate : Die CBCT zeigte keinen direkten Kontakt zwischen den Wurzeln des Zahnes 38 und dem Canalis mandibulae. Stattdessen wurde eine scharf begrenzte, homogene radiopake Läsion von etwa 7,5 mm nachgewiesen, die in unmittelbarer Nähe des Kanals lag, jedoch von den Zahnwurzeln getrennt war und mit einer idiopathischen Osteosklerose vereinbar war. Die operative Entfernung der Unterkieferweisheitszähne verlief komplikationslos. Aufgrund des asymptomatischen Verlaufs sowie des Fehlens neurologischer Ausfälle oder aggressiver radiologischer Merkmale wurde auf eine Biopsie verzichtet und eine radiologische Verlaufskontrolle empfohlen. Diskussion : Der vorliegende Fall verdeutlicht die Grenzen der Panoramaschichtaufnahme bei der Beurteilung der Beziehung zwischen Unterkieferweisheitszähnen und dem Canalis mandibulae. Die CBCT ermöglichte eine zuverlässige dreidimensionale Darstellung und klärte die diagnostische Unsicherheit, die durch die Überlagerung einer idiopathischen Osteosklerose entstanden war. Eine sorgfältige radiologische Beurteilung und differenzialdiagnostische Abklärung sind entscheidend für eine sichere chirurgische Planung und zur Vermeidung unnötiger Risiken.
