Une hydrocéphalie chronique peut nécessiter une insertion d'un drain ventriculo-péritonéal (VPS). L'objectif de l'étude est d'identifier les facteurs prédictifs indépendants et établir un modèle prédictif de dépendance au drain. Méthodes: 324 patients atteints d'HSA ont été recrutés de manière prospective et consécutive dans l'étude @neuriST. L'analyse a inclus onze variables. Un modèle prédictif pour la dépendance au shunt a été développé sur la base d'une analyse multivariée des variables sélectionnées par l'analyse univariée. La performance des modèles a été mesurée et comparée au score CHESS. Résultats: Une hydrocéphalie dépendante du shunt s'est développée chez 38 (11,7%) patients. L'hémorragie ventriculaire, l'échelle WFNS, le grade de Fisher, la taille initiale du troisième ventricule, l'emplacement postérieur de l'anévrisme et le placement du drain ventriculaire externe (EVD) ont été associés à l'hydrocéphalie dépendante du shunt. Le placement d'un EVD et l'emplacement postérieur de l'anévrisme étaient des prédicteurs indépendants sur l'analyse multivariée. Les classificateurs basés sur le score CHESS, le placement EVD seul ou en l'association avec la localisation de l'anévrisme ont montré des performances similaires. Conclusions: Le placement initial d'un drain ventriculaire externe semble être le meilleur facteur prédictif de la dépendance au shunt. Les modèles a 1,2 ou 5 variables (score CHESS) ont des performances prédictives similaires. Objective: Hydrocephalus is a frequent complication of subarachnoid hemorrhage (SAH) following the rupture of intracranial aneurysms. Chronic hydrocephalus develops in 6 to 40% of patients and may require insertion of a ventriculo-peritoneal drain (VPS). The objective of the study is to identify the independent predictor and establish a predictive model of shunt dependency. Methods: 324 patients with HSA were recruited prospectively and consecutively as part of the @neuriST study. The analysis included eleven variables. A predictive model for shunt dependence was developed based on a multivariate analysis of the variables selected by the univariate analysis. The performance of the models was measured and compared to the CHESS score. Results: Shunt-dependent hydrocephalus developed in 38 (11.7%) patients. Ventricular hemorrhage, WFNS scale, Fisher grade, initial size of the third ventricle, posterior location of the aneurysm and placement of the external ventricular drain (EVD) have been associated with shunt-dependent hydrocephalus . The placement of an EVD and the posterior location of the aneurysm were independent predictors on the multivariate analysis. Classifiers based on the CHESS score, EVD placement alone or in combination with the location of the aneurysm showed similar performance. Conclusions: The initial placement of an external ventricular drain appears to be the best predictor of shunt dependence. Models with 1,2 or 5 variables (CHESS score) have similar predictive performance.
